Submit a Cancellation Request

 

*The submission of this request form does not guarantee cancellation will be processed. You will receive a response within 1 business day via email regarding the status of your request. 

 

Membership Cancellation Policies:   

 

MONTHLY MEMBERSHIPS

Cancellation requests must be submitted at least five (5) days prior to the date at which you wish the membership to be cancelled.  You are responsible for knowing your membership billing date. If you do not know it, you may submit a request in writing to info@riseupfitnessSB.com for your membership information. If your membership cancellation request is not submitted five (5) days prior to your billing date, you will be billed for one additional month. 

 

MEMBERSHIP CONTRACTS (3-Month, 1-Year)

Cancellation requests must be submitted at least five (5) days prior to the date at which your contract renews. You are responsible for knowing your contract renewal date. If you do not know it, you may submit a request in writing to info@riseupfitnessSB.com for your membership information.  If your contract cancellation request is not submitted five (5) days prior to your contract renewal date, your contract will renew automatically for another cycle. If you wish to cancel your membership mid-contract, you may be subject to early termination fees pursuant to your initial paperwork.
